At Landing Hand Care LLC, we understand the complex needs of adults facing serious mental health challenges or substance use disorders. Our Home and Community-Based Services for Adult Mental Health (HCBS-AMH) program offers a comprehensive range of support tailored to each individual's unique physical, psychological, emotional, and social needs.

What is HCBS-AMH?
The HCBS-AMH program provides home and community-based services for adults with serious mental illness (SMI). Our services are person-centered, supporting individuals to:

Recover from mental health challenges.

Live independently.

Achieve stability and experience meaningful community integration.
Program Eligibility Criteria
To qualify for HCBS-AMH services, individuals must meet the following criteria:
Initial Eligibility:

Be an adult (18 years or older) with a diagnosis of serious mental illness (SMI).

Meet financial eligibility at or below 150% of the Federal Poverty Level (FPL).

Meet state-defined needs-based criteria.
Additional Eligibility (during the 3–5 years prior to referral):

In-Patient Psychiatric History:
-
Resided three or more years (cumulative or consecutive) in an inpatient psychiatric hospital and is Medicaid eligible.

Psychiatric Crises and Arrests:
-
Experienced two or more psychiatric crises and four or more arrests.

Frequent ED Visits:
-
Had two or more psychiatric crises, 15+ emergency department visits, and active Medicaid.
